The Registrar shall return incorrect and/or incomplete forms, forms with illegible data, or forms with words crossed out or erased. Once accepted, this application shall be the document of record. The ISWS reserves the right to cancel for cause, the registration of this dog and/or its descendants. Misrepresentation of this application is one example to cancel for cause and may result in loss of ISWS privileges.

Be sure to put a notice of what you are paying for on the PayPal comments: Litter Registration for: Names of parents, your name, email, phone, etc.
